What is MEDSort.com?

       

       

Medsort.com is a comprehensive medication list and personal health record (PHR) management system.   It is designed to help patients keep an up-to-date, self-reported list of their medications, allergies and other health information and automatically relay this to their health care providers.  Medsort.com provides an interactive, secure medium to link patients and physicians, improve quality of care, and reduce medical errors.  

       

       

MEDSort's History:

       

MEDSort.com was founded by a team of physicians and pharmacists who understand the complexities of our fragmented healthcare system, where accessibility to patients' health information is often limited.  Our mission is to improve the quality of patient care by allowing patients to store and manage their medication list and other health information online and to make that information available to healthcare providers.  In fact, MEDSort.com goes beyond simply making this information available to healthcare providers; a copy of the patient's most recent medication and allergy list is automatically sent to their health care provider(s) every time there is a change in the content of this list.  Medsort.com empowers patients by giving them complete control over the content of and access to their vital and private health information.   The information will be stored securely and no one will have access to this confidential information without the explicit permission of the patient.

       

Why should I use MEDSort.com?

       

Patients are usually surprised to find out that their physicians often do not know exactly what medications they are taking.  Frequently the medications that a patient is taking do not correlate with the medication record that the physician has in the patient's chart.  This is because physicians frequently add or remove medications from the list, or even change the dose or the frequency of the medications.  This is often done over the phone or on weekends, or by other covering physicians, hospitals or healthcare providers where it is not reflected in the patient's chart. 

       

Another important factor is that patients often have several physicians who prescribe different medications without automatic communication to patient's other providers.  These factors can potentially create confusion for physicians and other healthcare providers who need to know the exact up-to-date medications their patients are taking.

       

There are many situations where a patient's medical chart is not easily available.  For example, when patients change doctors or are referred to new physicians, often the new doctor does not have the old medical records at the time of the first visit in the office.  MEDSort.com allows patients to have an updated medication and allergy list online where it is easily and immediately available at all times.  You have the ability to fax this information to your new doctor before or on the day of your doctor's visit.

       

In addition, patients using MEDSort.com will be able to carry an emergency access card with them where ever they go.  In case of an emergency, if you are traveling or have to seek medical attention in an urgent care or emergency department, you can have any healthcare provider look up your medication list online using your emergency access card. 

       

Sending my medication list to my doctors:

       

An important feature of MEDSort.com is that it allows you to fax your medication and allergy list to your health care providers.  You can send your medication list to any of your doctors at any time.  In addition, you can schedule to fax your medication list at a future date to any of your physicians.  For example, if you have a doctor's visit next month, you can schedule your most up-to-date medication list to be faxed to your doctor on the date of your visit.  On the day the fax is sent, it will include your most updated medication list at that time.

       

Another important feature of MEDSort is the option of automated faxing.  By choosing the automated fax feature, anytime you make any changes to your medication or allergy list, an updated fax will be sent to your doctor.  This ensures that your health care providers will always have your most recent and accurate medication and allergy list in your medical chart.

MEDSort.com is designed with the specific intent of improving your quality of healthcare by minimizing medical errors.  Taking the wrong medication or the wrong dose of a medication can have serious adverse health consequences.  With this in mind, MEDSort.com has created a unique system whereby your health care providers can verify the accuracy of your medication list.  This is done in two ways.

       

The first requires minimal effort on the part of your physician.  By simply faxing your current medication/allergy list to your physicians, they can check to make sure that you are taking the right medications with the right dose and at the right frequency.  If your doctors find a major error on your medication list, they can contact you to correct the potentially dangerous mistake.  In addition, they can keep the list you send them in your chart so they always have access to the most up-to-date medications you are taking.

       

The second way your doctors can verify the accuracy of your medication list is by logging on and viewing your medication list online.  Once your doctor registers at MEDSort.com, an automatic link is established whereby they are able to view and interact with your self-reported medical information.  Your doctor can then verify your medication list online.  The next time you log on, you can see which medications on your list are accurate and which ones your doctor feels are inaccurate.  If you do not want a registered physician to be able to view your information online, then simply remove that physician's name from your physician list.

       

Remember, the medications you are actually taking do not always correlate with the medications your doctors think you are taking or should be taking.  By using MEDSort.com, your doctors can reconcile this difference and avoid potentially dangerous medication errors.

       

Physician Access:

       

As a physician, you can take advantage of MEDSort.com with no extra effort on your part.  As practicing physicians, we understand the time constrains of a busy practice.  We also understand the frustrations of trying to reconcile your patient's most updated medication and allergy list when there is incomplete or conflicting information available.

       

For those patient's who have a MEDSort.com account, you will automatically receive the most recent version of their self-reported medication list which will be faxed to you by your patient.  This will ensure accurate and updated chart data and will allow you to know if your patients are taking what they should be taking.

       

On the other hand, you can register with MEDSort.com as a provider and have immediate access to the information about all of your patients who are members.  As a registered physician, you can view, verify, and add medications to the patient's list with an interactive link.  Furthermore, your verifications are reflected on the patient's medication list when it is sent to you or any other provider.

       

In addition, registered providers are listed in a searchable database so your patients can find you when registering.  You also have access to labeling information on most medications and a state of the art medication interaction and pill picture database which will be available shortly.

       

Physicians Using EMR (Electronic Medical Records):

       

If your office uses an electronic medical records system, you can take full advantage of MEDSort.com services.  You can use your patient's self-reported medication list to reconcile with the patient's electronic record.

       

We are currently working to allow direct download of this information to your software using the current health information exchange standards.  As registered providers, you are poised to take full advantage of this feature when available. 

  

       

MEDSort Security:

     

We take the security of your confidential health information very seriously.  Any information you store on MEDSort will be kept securely and will remain confidential.  As patients, you will have complete control over your confidential health information.  No one will be able to view your medical information without your consent.

       

MEDSort Funding Disclosure:

     

MEDSort is privately funded. This means that there are no ties to any pharmaceutical organizations or other third party sponsorships.  MEDSort operates independently to bring you the services provided.  There may be third party advertising on some pages at medsort.com.
